Patient Centered Medical Home
Patient Centered Medical Home is a program established by the National Committee of Quality which awards recognition to primary care offices who strive to put the patient’s needs first, coordinate their care and help them stay as healthy as possible.
The care delivered to patients needs to meet the following five requirements, set by the U.S Agency for Healthcare Research and Quality:

We are striving to gain this accreditation by the National Committee of Quality and Assurance. Together, we share a commitment practice evidence based medicine, while caring about our patient outcomes and satisfaction.
A Better Model of Care
Studies show that the patient-centered medical home improves patient care and satisfaction and reduces cost. This model of care brings together a team of health professionals, coordinated by your primary care physician. The team works collaboratively to provide high-quality integrated care, easy access, and good communications—all of which play an important role in keeping you healthy.
Join our Patient Advisory Council and Help Enhance Quality
As part of our commitment to quality improvement, we continually assess our performance, identify areas for improvement, make changes—and assess again. We involve patients in these quality monitoring and improvement efforts as members of our Patient Advisory Council. It literally gives patients a seat at the table and an opportunity to impact the planning, delivery, and evaluation of patient care.
Monday – Friday:
8:00 a.m. to 4:30 p.m.
Extended hours, by appointment:
7:30 a.m. to 8:00 a.m. and
4:30 p.m. to 7:00 p.m.
